Understanding ABGs: A Quick Guide

An arterial blood gas, or ABG, is one of the most powerful diagnostic tools we have for understanding a patient's oxygenation, ventilation, and acid-base balance — all from a single blood sample. Every ABG gives us five key values: pH, PaCO₂, HCO₃, PaO₂, and SaO₂ — and each one tells us something specific about what is happening inside the patient's body right now. The lungs control carbon dioxide, and this directly affects pH. When CO₂ rises, the blood becomes more acidic and pH falls. When CO₂ drops, the blood becomes more alkaline and pH rises — they always move in opposite directions. Respiratory acidosis happens when a patient hypoventilates and retains too much CO₂. You will see a low pH and a high PaCO₂, while bicarbonate stays normal if there has been no time for compensation yet. Respiratory alkalosis is the opposite — the patient hyperventilates, blowing off too much CO₂, which drives the pH upward. Anxiety, pain, fever, and early hypoxemia are all classic triggers you should recognize immediately. Metabolic acidosis means bicarbonate is too low, dropping the pH. The lungs try to help by increasing ventilation to blow off CO₂ — this compensation produces the deep, rapid Kussmaul breathing pattern seen in diabetic ketoacidosis.
