Meet John, a 72-year-old man who has just been told his stomach cancer is no longer curable, and that no treatment can save his life. John is fully conscious and mentally capable of making his own medical decisions — his mind is clear, and his wishes are his own. John tells his doctor directly: he does not want painful surgery or aggressive chemotherapy — he wants palliative care and to be comfortable at home. But his adult children strongly disagree. His daughter pleads with the doctor not to give up, and his son demands that every possible treatment be tried. The children are driven by love and fear — they are terrified of losing their father, and they believe stopping treatment means abandoning him. The doctor is now caught in the middle, pulled in opposite directions — the patient wants comfort, while the family demands aggressive intervention. Both sides refuse to give in, so what should the doctor do? In medicine, a patient with full decision-making capacity has the legal and ethical right to choose their own care — and that right must be respected.
